Eleazar Tayag, PT, DPT, MPH, an exemplar of hard work and dedication, has built an illustrious career in the domain of physical therapy. This eminent professional is a Board-Certified Clinical Specialist in both Geriatric and Orthopaedic Physical Therapy, a testament to his commitment to improving patient outcomes and quality of life. His skills extend to home health and home modifications, thanks to his certification as an OASIS Specialist – Clinical (COS-C) and Advanced Competency in Home Health. Dr. Tayag leads by example in his roles as CEO of Imperial Home Health and Optimum Care Provider and COO of Caring Shield Hospice, Inc. His leadership is rooted in his deep academic background; he secured his bachelor's degree in physical therapy from the Lyceum Northwestern University, where he graduated With High Distinction. Later, he gained a master’s degree in public health from the University of the Philippines before receiving his Doctorate in Physical Therapy from EIM Institute of Health Professions in the United States. Throughout his academic and professional journey, Dr. Tayag has consistently demonstrated his outstanding abilities, ranking highly in numerous licensure and civil service exams. Prior to his move to California in 2005, he served as Dean of the College of Physical Therapy of the University of Pangasinan, influencing a generation of physical therapists. Beyond his professional roles, Dr. Tayag has made significant contributions to the non-profit world. He's the current President of the FUTURE Foundation Global, a platform promoting global Filipino Physical Therapists. He moderates the Academy of Filipino-American Physical Therapists, an association of over 8000 members, and reviews manuscripts for the Philippine Journal of Physical Therapy. Continuing his journey in academia, he serves as a faculty member for APTA Home Health’s Advanced Competency in Home Health and a Clinical Instructor for the University of St. Augustine for Health SProfessional Education
